All clients are required to submit Proof of Residency in the form of proof of address for adults and proof of children for children. These documents need to show your name and current address. If you wish to submit your proof of residency before coming to the Center, please upload it below. You are also welcome to bring these documents to your first time at the Center to show a staff member. If you have multiple people in your household, please submit this form as many times as needed for all members. Please submit one proof of residence per person.
